By submitting this form, I agree to the following:

I acknowledge that my property, including, without limitation, any art or other items on display (hereinafter referred to collectively as “Property”) may be damaged, lost or stolen during the exhibition of and/or during the unpacking, packing, or transportation of and I acknowledge and understand the risk involved by allowing such property to be displayed.

I understand that the GEAG (Gateway East Artist Guild) does not insure the Property and if I so choose I must acquire my own insurance.

I agree to waive any personal injury, property damage and/or property loss claim, now or in the future, that I may suffer as a result of any exhibition of my Property. I agree to indemnify and hold harmless GEAG, its Board, and any employees and/or designated representatives, from any personal injury, property damage and/or property loss claim, now or in the future, that I may suffer or cause as a result of the exhibition of my Property.

I hereby represent and warrant that I have read this Artist Loss/Damage Waiver in its entirety and fully understand its contents. I agree to this waiver voluntarily and of my own free will. I release and hold harmless the GEAG, its Board, employees and/or representatives from and against any and all claims of injury, loss or damage relating to the above provisions.
